 PARENT AND CHEERLEADER COMMITMENT FORM

Bridgewater Badger Cheer/Pep Squad Parents

  1. As a parent, I need to make time to get my daughter to practices and games on time.  If needed, I will make arrangements with someone else to drop-off and pick-up my child.  Coaches and Advisors may not give girls a ride to practices and games.
  2. Fundraising is a big part of cheerleading.  I understand that the Board will be asking me as a parent to be involved.  This includes chaperoning dances, working in the apparel tent and selling tickets and donating items for the basket raffle.
  3. All parents must sign and return a completed medical info/release form and a Waiver and Release of Liability.  A child is not allowed to attend practices or games until both forms are returned.
  4. Full Day Camp is mandatory and will be held the week of August 18th- days vary depending on squad.
  5. I have read the 2008 Rules and Regulations posted on the website and agree to all.
